Planning your appointment
For every visit, please:
- Arrive 30 minutes prior to your visit.
- If you are over 9 minutes late, you will be asked to reschedule.
Bring:
- Photo ID
- Insurance Card
- Co-Payment
- A translator, if necessary
Helpful Items
- Medical Records
- Copies of X-Rays/CT/MRI scans
- List of conditions and illnesses
- List of medications or pill bottles
- Any forms you need completed (7 day turnaround time)
- A list of any questions you might have.
